Enhance Scheme

Our Enhance scheme is up and running! As one of fourteen organisations in the city of Leeds to be a delivery partner for the Enhance Programme, run by Leeds Older People’s Forum and Forum Central, we have the opportunity to focus on tailoring our services to the needs of those who are being discharged from the hospital. By linking Neighbourhood Teams with third-sector organisations, the aim is to avoid fewer readmissions.

Our Support Team will focus on the first few days of transitioning back into the home, during which our “Home Welcome Packs” will be distributed to clients, any essential shopping will be ordered and delivered, the home is well heated and able to accommodate hazards prevention equipment, necessary transport to appointments and for deliveries is arranged. Following the first few days of being back at home, these calls will be gradually reduced depending on individual client need and we will liaise with our Neighbourhood Teams to organise referrals to befriending and other support services.

Care & Repair look forward to delivering on the outcomes of the project and gaining experience  from the project’s ‘test and learn’ approach.
