Dementia Care


The Dementia Support Worker delivers the Home Plus Leeds Service as well as providing advice guidance and support for clients living with dementia and where relevant their carers.

This additional focus aims to assist people living with dementia by assessing for and providing home-based interventions to improve their health and wellbeing and facilitate more independence and control over their lives.

Outreach support is provided through home visits and holistic assessments of how routines and activities which form the day to day lives of each client living with dementia can be supported and enhanced.

In addition to providing dementia friendly adaptations, advice on making homes dementia friendly is available through the use of Care and Repair’s extensively researched guide ‘Living Well with Dementia at Home’. Learn how to make your home Dementia friendly

Advice information and support is available on many aspects of dementia care including support and respite for carers and referrals to other appropriate support agencies health professionals and community organisations. Practical solutions to help people living with dementia maintain safety and independence such as coloured rails, or enabling equipment to support dressing, cooking, eating and memory aids can be advised upon.


Dementia Action Alliance:

We are part of the Dementia Action Alliance of Leeds, providing 8,500 people living with dementia across the Leeds area with support tailored to their needs. As one of over 900 organisations that want to make a difference to the treatment of, attitudes around and overall quality of life of those living with dementia and their carers, we are committed to:

Raising awareness of dementia to our clients, including both those who work with us and those who use our services

Focusing internally on the knowledge, skills and understanding of our staff and developing our staff training to include dementia awareness

Ensuring that the services we provide serve the needs of those living with dementia, for instance, providing minor and major adaptations that will provide a better environment for someone with the condition.
